  • Symptom and Medical History Summary
    Because ITP has similar symptoms as other diseases affecting platelet count, it’s important to tell your doctor about all your symptoms and your complete medical history. Print the Symptom and Medical History Summary, fill it out, and bring it to your next doctor appointment.

  • Nplate® Dose and Platelet Tracker
    Tracking your platelet counts helps you know if you are meeting your ITP treatment goals. Your doctor will use a blood test regularly to check if your platelet count is within a range sufficient to reduce the risk of bleeding. If you are taking Nplate®, your doctor will adjust your dose if necessary.1 Print this sheet and use it to keep track of your platelet counts and Nplate® dosing regimen.
